Abstract
This capstone project is designed to increase the awareness and knowledge of health and human services available for use by the homeless, including those with a psychiatric disability, in Monterey County and specifically MCHOME clients. It will serve as a tool for the homeless community to alleviate potential barriers to receiving services. This resource guide will enable homeless to take more control over their lives during their transition from homelessness to permanent housing.
