Abstract
The project aimed to address the lack of case management support for clients in the In-Home Supportive Services (IHSS) program by improving IHSS hour utilization and enhancing access to essential resources among seniors. The focus was on seniors aged 65 and above, primarily monolingual and lacking technological literacy, residing in the South County area, particularly in Watsonville. The project ensured that four clients received case management services, with at least three of them demonstrating increased utilization of IHSS hours within three months. Additionally, clients gained greater access to resources through relevant referrals.
The implementation process included comprehensive assessments, weekly home visits, consistent communication with clients, collaboration with community providers, and ongoing documentation. These strategies ensured that clients received the support needed to navigate the IHSS program effectively.
As a result of this project, four seniors received case management services, which included assistance with the Electronic Visit Verification (EVV) system and connections to multiple resources. For two of these seniors, their caregivers were able to receive payment for hours they had worked but had not yet been compensated for, with backlogged hours ranging from three months to 1.5 years. The other two seniors, whose caregivers were providing care but were not yet enrolled in IHSS, received assistance with the enrollment process. This will allow them to claim previously worked hours and ensure they receive future payments as well.
Recommendations for next steps include expanding the number of clients participating in the project and sustaining collaboration with community stakeholders to connect seniors with appropriate agencies offering resources to enhance their quality of life.
