Abstract

Poorly managed asthma is a common cause of unnecessary medical encounters for children and adults. Monterey County Health Department’s (MCHD) Asthma Case Management Program conducted by MCHD’s Public Health Regional Teams was created to deliver in-home asthma self-management education for the parents of young children and teens under the age of 17. The purpose of the program was to improve asthma self-management through the use of education, monitoring, and medical follow-up visits. An internship at MCHD Planning, Evaluation and Policy Unit provided an opportunity to evaluate the effectiveness of the program through pre- and post- survey analysis. The program evaluation was based on three-month post-intervention survey results which appeared to indicate that the program contributed to increased asthma knowledge and improved functional outcomes.
