Poorly managed asthma is a common cause of unnecessary medical encounters for children and adults. Monterey County Health Departmentâ€™s (MCHD) Asthma Case Management Program conducted by MCHDâ€™s Public Health Regional Teams was created to deliver in-home asthma self-management education for the parents of young children and teens under the age of 17. The purpose of the program was to improve asthma self-management through the use of education, monitoring, and medical follow-up visits. An internship at MCHD Planning, Evaluation and Policy Unit provided an opportunity to evaluate the effectiveness of the program through pre- and post- survey analysis. The program evaluation was based on three-month post-intervention survey results which appeared to indicate that the program contributed to increased asthma knowledge and improved functional outcomes.
